Job Summary
We currently have a permanent Senior Community Nurse opportunity within our Leek PCN community Nursing team based at Leek Moorlands Hospital. Community nursing teams deliver holistic, complex care to housebound patients within their own home environments. You will be responsible for leading the community nursing team, managing the caseload and supporting staff development. You will work alongside the leadership team to ensure proactive and effective care delivery, liaising and working with the multidisciplinary team to ensure individualised and evidence- based care to our patients.

Main Duties of the Job

• You will be responsible for allocating patient visits within the skill set for the community nursing team. You will work alongside the Lead Practitioner to manage the caseload, including reviewing demand and capacity.
