The Children and Young People's Continuing Care (CYPCC) service leads on the assessment, case management and oversight of the NHS National Framework for Children and Young People's Continuing Care across Kent and Medway. The service assesses, develops and oversees personalised care packages for children and young people with complex health needs in the community, working in partnership with social care, education and wider system partners as part of a multidisciplinary network.
