Senior / Staff Nurse (Chronic Disease Management) - Geylang / Kallang Polyclinic
  We are looking for dynamic nurses to work with multi-disciplinary team to manage patients with chronic conditions. You will provide patient education and teach self management for patients with chronic conditions. Relevant training and development opportunities will be provided to successful candidates.
  Requirements:

Registered with Singapore Nursing Board

Has 2 to 3 years of nursing experience in chronic disease management

A Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred

A good team player

Possess excellent interpersonal and communications skills

Proactive and self-motivated

Working location: Geylang / Kallang Polyclinic

** Candidates with relevant experience could be considered for Senior role.
