**Overview**

Summary: Assist in assessing and identifying resident and family needs and outside resources to meet those needs. Areas of assistance may include physical, mental, psychological and/or social as it relates to aging and moving in/moving out.

**Responsibilities**

1. 1. Initiates and completes assessment, interviewing residents, families and others in order to evaluate current condition and identify current or potential needs and services.2. Provide general case management and referral services3. Coordinate communication between residents, family members, medical staff, administrative staff, or regulatory agencies.4.
