Position Overview

Job Responsibilities:


  • Performs Patient assessments:


    • Accurately assesses patient’s appropriateness for home care, learning needs and progress toward goals.

    • Collaborates with patient, family, and physician to develop the plan of care for the patient.

    • Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated.



  • Maintains safe environment for patients and co-workers:

    • Assesses social environment and adjust or provide education as needed.

    • Assesses for potential clinical risks and adapts program as indicated.

    • Follows universal infection control measures.



  • Implements treatment program to achieve designated goals:

    • Administers occupational therapy services per physician orders.

    • Instructs in communication and safe swallow, cognitive training as...
