This is an exciting opportunity for a Social Care Practitioner (SCP) to join our team as we continue with our multidisciplinary offer for young people transitioning between children’s services and adults. We are a multi-disciplinary team working with Social Workers, Occupational Therapists and Social Care Practitioners. 

In Bristol, we are committed to hearing the young person’s voice, and we have a newly created Standard Operating Procedure (SOP) for our Transitions Hub model.

The Transitions Hub is our city-wide commitment to supporting seamless transitions for young people progressing to adulthood, and within the Hub, the Young Adults Transitions Service have a key role in transitions casework and in providing early support.
