Maximising Independence Team – Social Care Practitioner

A new exciting opportunity for an experienced social care practitioner to join our innovative team has arisen.

Would you like to work as part of a new multi-disciplinary team, working with people who have complex and enduring mental health challenges, who are supported by adult social care?

The maximising independence team will help people to become more independent by reassessing people currently supported in the Adult Mental Health Social Care Service , and supporting them move on from long-term provision via an MDT approach to (re)gaining abilities and skills, set enablement goals, and take advantage of opportunities to grow and develop their lives.
