Job Details

We are looking for a dynamic, enthusiastic and motivated Social Work Assistant to join the Social Work Hospital Discharge Team (West) on a temporary basis for 12 weeks. The team works with adults who have physical and/or mental health needs, their families and carers promoting a person-centred approach to discharge planning. The West team covers Victoria Hospital and Queen Margaret Hospital as well as Fife residents in hospitals such as Lothian and Falkirk. 

The successful candidate will work collaboratively with social workers contributing to personal outcome focused assessments as part of discharge planning. They will engage with individuals, families and carers to address some of the practical barriers to discharge whilst promoting their rights and strengths to achieve better outcomes.
