About the Role
To provide social work services to patients and their whanau of Te Whatu Ora- Southern by:

* Undertaking social worker assessments, interventions and evaluations
* Working with the MDT ensure safe care plans
* Protecting vulnerable patients from harm because of abuse and neglect
* Collaborate community service providers to deliver wrap around supports for patients
