Summary The social worker serves as a member of an interdisciplinary treatment team contributing to overall primary healthcare services to Veterans assigned to a primary care team. The social worker provides clinical counseling, psychosocial assessment, and case management services to eligible veterans and their family members to meet biological, psychological, social, and environmental needs.
