What will your responsibilities be?
Assess patient needs to support emotional, social, and practical well-being during hospitalization.Coordinate discharge planning by arranging follow-up care, rehabilitation, or community services.Provide crisis intervention and emotional support to patients and families during medical emergencies.Advocate for patient rights and help navigate healthcare systems, benefits, and legal protections.
