Reporting to: Head Community Care / Senior Social Worker / Centre Manager
Job Purpose To support the psycho-social well-being of seniors through casework, care coordination, and social support activities, working closely with the Centre team to promote active ageing, social connectedness, and early intervention for vulnerable seniors in the community.
Key Responsibilities 1. Casework and Care Coordination - Conduct intake and needs assessments for seniors referred to the AAC.
- Develop and implement individual care plans in consultation with the Senior Social Worker and Centre team.
- Provide emotional support and basic counselling to seniors with adjustment, loss, or family-related issues.
- Coordinate with AIC, FSCs, healthcare providers, and social service agencies for referrals and resource linkage.
