Aberdeen City Out of Hours Nursing Band 6 Senior Community Nurse
There is an opportunity for Band 6 Senior Community Nurses to join our Aberdeen City HSCP Evening and Night Community Nursing Unscheduled Care Team.
The Senior Community Nurse works with and supports Band 5 and Band 3 colleagues delivering a person‑centred clinical nursing service that prioritises end‑of‑life care and symptom control for patients in their own homes and plays a vital role in supporting admission avoidance. The role includes triage and management of unscheduled nursing referrals from GMED, NHS 24, Hospital at Home and other professionals, and provides support for the District Nurse with team management responsibilities.
Deliver person‑centred nursing care to patients at home, triage unscheduled referrals, manage workload, provide palliative and end‑of‑life care, and support district nurse and team management.
