Job Title: Specialist Case Manager (Children and Young People Complex Disabilities)
Location: Home based with a requirement to cover clients within 2 hours from home address (Full UK driving license + own transport are essential, mileage is paid)
Salary: Starting from £48,500 per annum FTE
Status: Permanent (salaried or flexible hourly contracts available)
Working Hours: Part Time & Full time (37.5 hours) contracts available with a minimum requirement of 21 hours over 3 days
