**Job Description:**

Gather psychosocial information to participate in the development and implementation of a plan of care that provides extraordinary care for the patient while being sensitive to costs and resources. Work collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties. The SSW will execute these duties under direct clinical supervision.
**Position Details:**

**Shift** : 8am -4:30pm on Tuesday's and Thursday's. No weekend or holiday requirement but they can work on a holiday if it falls on the day of a scheduled shift, but would not be required to.

**Department** : Bbehavioral health and an access center

**Minimum Qualifications:**

+ State of Utah SSW license
+ Three years of experience working with Behavioral Health Patients in a behavioral health care setting.
