Overview Assist clients and their caregivers to acheive seamless transition from various stages of the care journey (e.g. Hospital to Home) by providing customer centric and holistic support services.
Responsibilities - Conduct screening referrals and assessment of potential clients and their home environment;
- Develop and perform Individualised Care Plan (ICP) involving caregivers and/or family members (if possible);
- Monitor clients' conditions and conduct periodical review of clients' ICP and conditions;
- Conduct financial counselling and provide advice on government subsidies available;
- Collect payment/s and issue receipt/s for services rendered;
- Manage hotline (on rotation basis);
- Assist doctors during home visits;
- Develop Discharge Plan for clients who no longer require our services;
