We have an exciting opportunity for Supportive Housing Case Manager's to join our Homes First Inner South team. 

As a Supportive Housing Case Manager, you will be part of the newly funded Homes First program, delivering services across Melbourne from our St Kilda office. The role involves delivering culturally safe three-year flexible support packages to participants who need support to sustain tenancies or move into stable housing.

Your day to day

A typical day will see you working with participants in an assertive outreach capacity to deliver case management and service coordination activities whilst liaising with other agencies, and community resources and services.
