MINDS is seeking a Transition Support Officer to provide hands-on support and guidance to clients transitioning between school, community hubs, and residential to community care settings.

You will collaborate with a multidisciplinary team to develop and review Individualized Development Plans (IDP) and engage caregivers to transfer skills into home and community environments. Strong communication and empathy are essential.
