Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break.
Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
Fit Test for N95 respirator within 365 days of start date and updated annually; no exemptions allowed.
