Job Responsibilities
Support safe transitions of care, including handoffs and care plan updates.
Work 36 hours per week with 12-hour shifts.
Provide patient and family education related to the plan of care and discharge needs.
Perform focused reassessments and escalate concerns through appropriate clinical channels.
Prepare patients for diagnostic tests or procedures and provide post-procedure monitoring as indicated. xhfqzwm
