Under the general direction of the Manager or designate, the Utilization Facilitator is responsible for
Facilitating and coordinating the movement of patients through the admission, transfer, and discharge process as required. Liaising between internal and external agencies on matters related to patient movement. Participating in the development of guidelines and educational strategies related to patient admission, transfer, and discharge. Participating in decision-making that results in an effective admission process and the optimal utilization of facility resources. Experience
