Under the general direction of the Utilization Manager and Facility Program Director(s) - Medicine/Family Medicine and in close collaboration with the Facility Vice President responsible for Utilization, Physician Bed Manager(s), and Clinical Manager(s), the Utilization Facilitator is responsible for
Facilitating and coordinating the movement of patients through the admission, transfer, and discharge processes primarily within the Adult Medicine/Family Medicine Program, ER Program, and other clinical programs as required. Liasing between internal and external programs and agencies on matters related to patient movement. Participating in the development of guidelines and educational strategies related to patient movement. Participating in decision-making that results in an effective admission process and the optimal utilization of Program and facility resources. Experience
