Your tasks

The rehabilitative assessment period begins based on the assessment of service needs, planned discharge from hospital, or concerns from the client or their relatives. You will carry out intensified and comprehensive home care, focusing on supporting the client’s functional capacity and independence through rehabilitative activities. During the period, you will work closely with a multidisciplinary team and participate in an evaluation at the end of the period, where, together with the client and their relatives, you will define personalized services to support and ensure safe living at home. The work is independent, diverse, and variable, with strong support from the team and supervisor.
