ESSENTIAL FUNCTIONS OF POSITION: • Ensure all residents are assessed weekly to evaluate the condition of their skin; identify risks and develop appropriate interventions and required documentation. • Measure all skin areas weekly and document. • Initiate and update all care plan information regarding skin. • Follow up with physician to report any resident skin problems that arise and transcribe and follow up with any new orders. • Perform skin and related treatments on all residents as assigned, according to Physician’s orders and in accordance with professional standards. • Communicate with the physician, residents, family members, charge nurse, other interdisciplinary team members, and facility management regarding resident status as appropriate. • Chair the Skin Management meeting on a weekly basis and provide needed information to the inter-disciplinary team to develop appropriate interventio...
